Q. What services are offered at your office?
Milan Medical Group has a family medical doctor for all your healthcare needs. Should you need a routine physical, medication refill, stitches, or lab work our physician is here for you. Our clinic also has a licensed chiropractor, physical therapist, and massage therapist to assist with your rehab needs.

Q. How do I make an appointment?
You may schedule an appointment by calling our office at 309-787-2600.

Q. What do I need to bring to my appointment?
When visiting the office for the first time please bring your health identification card and co-payment, if applicable. All applicable co-payments are expected at the time services are rendered. You will receive a bill if the services provided are not covered under your health care plan.

To speed up the process of your initial appointment you may download and print your health history forms to fill out prior to visiting the clinic. Please refer to the clinic forms section of our website. If you are unable to fill out these forms prior to your appointment, please allow a few extra minutes before your scheduled appointment to complete your paperwork and give us the chance to become acquainted.

Q. What if I have to cancel my appointment?
Please contact our office for any cancellations, 309-787-2600. We ask that you give us as much notice as possible so that other patients can utilize that time.

Q. Can I see you even if you do not participate with my health insurance?
Even if we do not participate with your health insurance, we are happy to provide your care. You will be responsible for payment for your services. Please contact our office for affordable payment plan options.

Q. Do you do routine GYN care, including Pap tests?
Our physicians are well trained in routine care of women and would be happy to assist your needs.

Q. Can I get urgent medical care, such as stitches, at your office?
Our physician is qualified and ready to assist you with your urgent care needs.

Q. What if I need access to medical care after the office is closed?
Our office is open Monday through Friday 8:00 am to 6:00 pm and Saturday and Sunday 9:00 am to 2:00 pm. Should you experience an emergency after office hours, we encourage you to go to your local emergency room or urgent care center. We would be happy to follow up with your health needs upon normal business hours.

Q. Is there anyone I can call with questions if I’m not sure whether I need to go to the hospital or not?
Trinity hospital offers a local My Nurse number for anyone to utilize with general health care questions. That number is 309-779-2000 or 1-877-242-8899.

Q. Do I need to have a doctor referral for physical therapy?
Although the state of Illinois does not require an order for physical therapy services, it is recommended. Insurance companies are more likely to cover rehab services with the request of a physician. You can bring in an order written by your family doctor, or you may see our medical doctor if you are anticipating needing therapy services.

Q. My doctor wrote me an order to go to physical therapy at a local rehab clinic, but I’d prefer to come to you because you are close to my home or work. Do I have the ability to choose where I get my therapy services?
Yes, absolutely. Although your physician or work may direct you to a specific clinic for physical therapy services, the state of Illinois says you get to choose what therapy office is convenient for you.
